House Calls
House Calls provides primary medical care to frail elders in their home. Eligible seniors include those who have limited access to care due to impaired mobility, cognitive impairment with a medical condition, limited financial resources, lack of health insurance or under-insured. We are like a “doctor’s office on wheels” making old fashioned house calls. Please call to find out if you or someone you know is eligible for House Calls.

St. Joseph Mobile Health Clinic
This Mobile Primary Health Clinic serves children and adults from low-income families who are without a regular primary care physician, and those who have difficulty obtaining affordable healthcare in their community.
St. Joseph Mobile Health Clinic transcends language, culture and access to care barriers by bringing healthcare directly to hose who need it most. The Clinic offers:
- Health screenings and well child exams
- Immunizations
- Treatment of minor medical problems
- Health and nutritional education
- Information and referrals

St. Joseph’s
Dental Clinic was founded to address the #1
need of children in our community: access
to dental care. The clinic serves children
ages 0-16 to provide basic, preventive,
emergency and comprehensive dental care with
a strong focus on prevention and education.
We provide emergency care to adults in the
morning on a first-come, first-served basis.The
Dental Clinic accepts the following insurance:
Medi-Cal, Denti-Cal, CHDP,
CaliforniaKids, Healthy Families. The clinic accepts those without insurance as well.

“Cultivando la Salud” — Cultivating Health Mobile Dental Clinic
A mobile community dental clinic provides basic, preventive, restorative and emergency dental care at a variety of sites throughout the county for agricultural workers, low-income families and special targeted efforts to serve children 0 to 5 years of age, pregnant women and special needs patients.

Home Sweet Home
Home Sweet Home enables frail elders to continue residing in their homes by providing them with personal care services. This allows seniors to remain independent and improve their quality of life. This program is available to frail seniors who lack access to care givers due to lack of financial resources or lack of family support. Please call to find out if you or someone you know is eligible.

"Promotores de Salud"
— Promoters of Health
The Promotores de Salud are community-based lay health & wellness promoters/educators who serve as a cultural bridge and a vital link to our community, working with families at a grassroots level. They provide resource connections and social supports to the community, to help both individuals and groups develop preventive strategies that address issues of primary concern to them. The Promotores also help families to enroll in Children's Health Insurance Programs, food stamps and other public benefits.

Children's Dental Disease Prevention Program
Mighty Mouth Health Education
A school-based children’s dental program to help prevent dental disease and promote dental health by teaching children to brush, floss and eat nutritious snacks. The program also includes school-based dental secreening, fluoride varnish, dental sealants and treatment.
