Hospice of Petaluma provides compassionate hospice care and grief
services to individuals and families facing life-threatening illness or
the death of a loved one.
Hospice Patient Care
Services is a comprehensive program of care provided in the patient's
home or other place of residence under the direction of the patient's
physician. Staff address the physical, psychosocial, emotional,
spiritual and bereavement needs of patients and their families.
Patients served in our hospice program include both children and adults
with diseases such as cancer, AIDS, and end-stage renal, heart and lung
disease.
Hospice of Petaluma provides grief support
services to the community at large. Services include counseling for
adults, teens and children, parent support groups, teen and adult
groups, and social gatherings for grief clients. Community education
programs about grief and loss are provided throughout the year.
Residents
of Sonoma County and West Marin County with a prognosis for life of six
months or less are eligible for our services. The patient or a
designated responsible person and the patient's physician must consent
to Hospice services.
No fees are charged to patient and
families for hospice services. Insurance reimbursement from Medicare,
Medi-Cal, private insurance or HMOs is received by Hospice of Petaluma
when available.
