Hospice services provide care and support for patients with life-threatening illnesses and their families. Our hospice programs provide clients with nursing, counseling, social work, home health aides and volunteers, grief counseling, support and education, and bereavement services.

Part of the St. Joseph Home Care Network of Northern California, the Sonoma County program is based in Rohnert Park. St. Joseph Home Care provides physical care, emotional care, and spiritual care for patients unable to easily leave their house for medical care.
With a personal visit from a healthcare professional, patients of our Home Care Network programs receive support services, medication and symptom management, and medical management for such conditions as AIDS/HIV and diabetes.
